Optimalisasi Kader Posyandu Lansia Mandiri RW VII Kelurahan Sukorejo dalam Manajemen Diabetes Melitus: Strategi Perawatan Kaki Holistik untuk Pencegahan Ulkus Kaki Diabetikum

This community service program aimed to optimize the role of Independent Elderly Posyandu cadres in RW VII, Sukorejo Village, in managing Type 2 Diabetes Mellitus (T2DM) through a holistic foot care strategy to prevent diabetic foot ulcers. Preliminary data showed that 27.9% of 312 elderly residents suffered from T2DM, 72.4% had uncontrolled blood glucose, and only 12% of cadres understood proper foot care, while 20.7% of elderly patients already had foot lesions. The program, named PRIMA KAKI, was implemented over one month involving 12 posyandu cadres and 30 elderly T2DM patients through four stages: cadre training, development of educational media, peer-support group formation, and provision of a foot examination kit, combined with routine home visits and a standardized foot-monitoring record system. Evaluation showed a 68.1% increase in the cadres' knowledge score, 88% of cadres able to perform the 10-step foot examination independently, a rise in elderly patients routinely performing self-foot-care from 12.6% to 78.5%, and a reduction in new foot lesions from 20.7% to 9.2%. These results indicate that a structured, cadre-based empowerment approach effectively strengthens community capacity to prevent diabetic foot ulcers and can be replicated in other elderly posyandu settings.
